<br />I. PURPOSE
<br />
<br />The purpose of this policy is to establish procedures for the operation and record
<br />keeping process for the Department’s Collision Review Board (CRB).
<br />
<br />II. POLICY
<br />
<br />The policy of the Surprise Police Department is to have all motor vehicle
<br />collisions involving Department vehicles, to include, leased, owned, rented,
<br />seized, or any other vehicles under Department control, reviewed by the CRB.
<br />Each review will be conducted within the guidelines of this policy for the purpose
<br />of providing a recommendation to the Chief of Police or designee as to whether
<br />the collision was “avoidable,”“unavoidable” or “avoidable but justified”. In
<br />addition, the CRB will also recommend if the collision was within or not within
<br />policy. The final decision will be determined by the Chief of Police or designee.

<br />III. COMPOSITION OF THE BOARD
<br />
<br />A. The Collision Review Board (CRB) will consist of the following personnel:
<br />
<br />1. Chairperson of the rank of Lieutenant
<br />2. Sergeant and Sergeant Alternate
<br />3. Traffic Service Officer and Traffic Service Officer Alternate
<br />4. Patrol Officer and Patrol Officer Alternate
<br />5. Detective and Detective Alternate
<br />6. Civilian and Civilian Alternate
<br />7. Professional Standards and Training Division (PSTD) Officer and
<br />PSTD Officer Alternate
<br />
<br />IV. SELECTION PROCESS
<br />
<br />A. The Chairperson will be appointed by the Chief of Police or designee.
<br />The Chairperson will generate an announcement every two years for
<br />memorandums of interest for open positions on the CRB. A list of names
<br />of those interested will be collected and the list provided to the Chief of
<br />Police or designee for final selection. The Chairperson will notify those
<br />selected in writing and will send a Department-wide email announcing the
<br />new Board members.

<br />V. PROCEDURE
<br />
<br />A. It is the responsibility of the on-duty supervisor to forward a copy of all
<br />associated Department reports, including the ADOT report, incident report
<br />and all other relevant documentation to the Professional Standards Unit
<br />
