PURPOSE <br /> <br />The purpose of this addendum is to: <br /> <br />A. Identify the review process that will be held following an emergency response incident. <br />B. Identify the personnel responsible for completing the necessary reports. <br /> <br />II. POLICY <br /> <br /> It is the policy of the Surprise Police Department to review all aspects of each emergency <br />response incident. <br /> <br />III. DEFINITIONS <br /> <br />A. Concluding Report - A report written by the Chief of Police based on submitted formal <br />reports. <br />B. Formal Report - A written analysis of an emergency response incident. <br />C. Informal Review - An informal debriefing with the Special Assignment Unit and <br />Negotiation Team Commanders and their team members. Discussion will include <br />individual/team problems encountered during the incident. <br /> <br />IV. RESPONSIBILITIES <br /> <br /> A. Command Officer <br /> <br /> 1. Submit a written report focusing on events and personnel not assigned to the <br />Command Post, Special Assignment Unit or the Negotiation Team to the <br />Incident Commander within two (2) calendar days of the incident conclusion. <br /> 2. Be available to attend any review of the incident upon request from the Chief of <br />Police. <br /> <br />B. Special Assignments Unit Commander <br /> <br /> 1. Submit a written report of those tasks assigned to the Special Assignment Unit to <br />the Incident Commander within two (2) calendar days of the incident conclusion. <br /> 2. Be available to attend any review of the incident upon request from the Chief of <br />Police. <br /> <br />C. Crisis Negotiation Team Commander <br /> <br /> 1. Submit a written report of those tasks assigned to the Crisis Negotiation <br />Team to the Incident Commander within two (2) calendar days of the <br />incident conclusion. <br /> 2. Be available to attend any review of the incident upon request from the <br />Chief of Police. <br /> <br /> <br />
