PURPOSE The purpose of this addendum is to: A. Identify the review process that will be held following an emergency response incident. B. Identify the personnel responsible for completing the necessary reports. II. POLICY It is the policy of the Surprise Police Department to review all aspects of each emergency response incident. III. DEFINITIONS A. Concluding Report - A report written by the Chief of Police based on submitted formal reports. B. Formal Report - A written analysis of an emergency response incident. C. Informal Review - An informal debriefing with the Special Assignment Unit and Negotiation Team Commanders and their team members. Discussion will include individual/team problems encountered during the incident. IV. RESPONSIBILITIES A. Command Officer 1. Submit a written report focusing on events and personnel not assigned to the Command Post, Special Assignment Unit or the Negotiation Team to the Incident Commander within two (2) calendar days of the incident conclusion. 2. Be available to attend any review of the incident upon request from the Chief of Police. B. Special Assignments Unit Commander 1. Submit a written report of those tasks assigned to the Special Assignment Unit to the Incident Commander within two (2) calendar days of the incident conclusion. 2. Be available to attend any review of the incident upon request from the Chief of Police. C. Crisis Negotiation Team Commander 1. Submit a written report of those tasks assigned to the Crisis Negotiation Team to the Incident Commander within two (2) calendar days of the incident conclusion. 2. Be available to attend any review of the incident upon request from the Chief of Police.
